Preliminary report On Qantas mid-air mishap released

Australian air safety investigators have released a preliminary report into a mid-air emergency concerning a Qantas jet.

The emergency occurred in a Boeing 747-400, en route from Hong Kong to Melbourne, which was forced to land in Manila after an explosion tore a hole in its fuselage.

Investigators have found the explosion was caused by an oxygen tank which punched a hole in the skin of the plane last month.

The Australian Transport and Safety Bureau has released a detailed account of how the passenger oxygen cylinder failed and then exploded in the aircraft hold.

It says the cylinder ruptured the fuselage, punctured the cabin floor and entered the cabin before falling to the cabin floor and falling out of the aircraft through the ruptured fuselage.

The ATSB says investigations are continuing into the functioning of the cabin oxygen masks and into the design and manufacture of the failed oxygen cylinder.

A survey of passengers is also being conducted.

Safest airliner gets poor report from safety investigators

What explosion? I have seen photos pointing to a rupture of a bottle or fitting (and the resulting missle action)but nothing that would indicate an explosion. An explosion implies that the oxygen released ignited and if that were the case then there wouldn’t be much to investigate.
The crew and passengers were extremely lucky that the oxygen release wasn’t as bad as it could have been.
Once the actual fault has been identified, blame and finger pointing can start. Conjecture and supposition presented as a line of explanation is witch hunting at best and downright lying at worst.
